Certificate of Insurance Request

A COI must never do anything more than serve as an affirmation that a particular policy existed for the named insured as of the date of the COI issuance.
 
1. The certificate is issued as a matter of information only and confers no rights to the certificate holder.
2. The certificate does not affirmatively or negatively amend, extend or alter the coverage afforded by the policies listed.
3. The certificate does not constitute a contract between the insurer, authorized representative or producer, and the certificate holder.
4. Should there be a cancellation before the expiration date, notice will only be delivered in accordance with policy provisions.
 
You Must Complete The Field For Client Type
 
If you are participating in a monthly subscription fee such as Staff Pro, you must select Subscription DB Staff Pro or the requested COI can’t be processed. For DB Staff Pro Clients, Click Here to Request Certificate of Insurance
 
If you aren’t sure what this means, you are probably a Traditional Client.
 
If you are the named insured, the answer is traditional insurance.
